Foundation Therapy was founded by Christopher Hicks, PT in August of 2013. The practice was initially opened as a direct payment for services provider. Since that time, Foundation Therapy has sought to provide individualized physical therapy services to individuals from Wise, Denton, Jack, Montague, Parker and Tarrant counties.

Foundation Therapy now accepts patients with most insurance providers. We are in-network providers for Medicare, Blue Cross Blue Shield, United Healthcare and hundreds of Workman’s Compensation providers nationwide. Direct payment for service rates are also available.

All physical therapy services provided in the state of Texas require referral from a qualifying healthcare provider.
